Online Hearing Checks vs a Clinical Hearing Assessment: What Is the Difference?

An online hearing check is a screening tool: it can suggest whether your hearing might need a closer look, but it cannot diagnose hearing loss. A clinical hearing assessment, carried out by an audiologist with calibrated equipment in a quiet room, measures exactly how well each ear hears, identifies the type of any hearing loss and tells you what to do next.
Think of an online check as a smoke alarm and a clinical assessment as the inspection that finds out what is actually happening. Both have a place. If an online check, or your own experience, suggests a problem, the next step is a professional hearing test.
What is an online hearing check?
Online hearing checks come in several forms: websites that play tones, smartphone apps, and "speech in noise" tests where you identify words or numbers against background noise. They are usually quick, free or low cost, and can be done at home.
Many are designed as screening tools, meaning their purpose is to sort people into "probably fine" and "worth checking further".
What is a clinical hearing assessment?
A clinical hearing assessment is a set of tests performed by a qualified professional. It usually includes:
- A discussion of your hearing difficulties, health and noise exposure
- A look inside the ears to check for wax, infection or other problems
- Pure-tone audiometry for each ear, by air and bone conduction
- Speech testing to see how well you hear and understand words
- Middle-ear tests, such as tympanometry, where needed

What are the key differences?
Calibration
Clinical equipment is calibrated, so the audiologist knows exactly how loud each sound is. With an online check, the loudness depends on your phone or computer, its volume setting and your headphones. Two people using different devices may get very different results from the same test.
Environment
Clinical tests are done in a quiet room or sound-treated booth. At home there is often background noise from air conditioning, traffic, other people or appliances, which can mask soft sounds and make results look worse than they really are.
What is measured
Most online checks give a general result. A clinical assessment tests each ear separately across a range of pitches, includes bone-conduction testing to find where the problem lies, and checks the middle ear. This is how the type of hearing loss (conductive, sensorineural or mixed) is identified. An online check cannot do this.
Looking in the ear
An online check cannot see wax, an ear infection or a problem with the eardrum. Some of these causes are treatable, and finding them matters.
Interpretation and next steps
A clinical assessment ends with a professional explaining your results, telling you whether you need a medical opinion and discussing options such as hearing aids. An online check usually ends with a score.
When does an online check make sense?
An online check can be helpful:
- As a first prompt if you are wondering whether your hearing has changed
- To encourage a family member who is reluctant to book an appointment
- As a general awareness tool, especially if you are regularly exposed to loud sound
Hearing loss is very common: according to the World Health Organization, over 5% of the world's population, around 430 million people, require rehabilitation for disabling hearing loss.
When should you go straight to a clinical assessment?
Skip the online check and book a clinical assessment, or see a doctor, when:
- You already notice clear signs of hearing difficulty in daily life
- One ear seems different from the other
- You have ongoing ringing in the ears
- You are considering hearing aids
- The concern is about a baby or young child, whose hearing needs age-appropriate testing
If your hearing has dropped suddenly, do not rely on an online check at all; see a doctor straight away.

Can a good online result rule out hearing loss?
No. A "pass" on an online check is reassuring but not conclusive, because the result depends on your device, headphones and surroundings, and because most checks do not test each ear thoroughly. If you are noticing difficulties, trust what you are experiencing and have a proper assessment.
When to see a specialist
Book a clinical hearing assessment if:
- An online check flags a possible problem
- Your online result looks fine, but you still struggle to hear
- Family members or colleagues have commented on your hearing
- You have never had a baseline hearing test
See a doctor promptly for sudden hearing loss, ear pain with discharge, ringing in one ear only, or dizziness with hearing loss.

Frequently asked questions
Can an online test diagnose hearing loss?
No. Online checks can suggest that your hearing may need attention, but they cannot diagnose hearing loss or identify its type. A diagnosis requires a clinical assessment with calibrated equipment and a professional who can examine your ears and interpret the results.
What affects the result of an online check?
The main factors are your device and its volume setting, the type and fit of your headphones, background noise in the room and how familiar you are with the test. Because these vary so much, results from different devices or rooms may not match. Treat any online result as a guide only.
What should I do if an online check flags a problem?
Book a hearing assessment with an audiologist so your hearing can be measured properly. Bring any screenshots or notes from the online check, along with a description of what you notice day to day. If the change in your hearing was sudden, see a doctor first and promptly.
